Peter Chien is an American entrepreneur and the co-founder and CEO of Kaigo Health, an artificial intelligence-powered outpatient care management platform focused on Medicare patients. The company was part of Y Combinator's Fall 2025 batch.[1]

Career

Chien attended the University of Illinois Urbana-Champaign, where he studied engineering.[2] Prior to founding Kaigo Health, he co-founded YourTable, a consumer social startup where he served as co-founder and CEO.[3]

Chien founded Kaigo Health to address operational inefficiencies in outpatient care management for Medicare populations. The company provides an AI-powered platform that automates and scales remote care management tasks, including post-discharge follow-ups, ongoing patient check-ins, and remote patient monitoring. Kaigo Health uses phone calls as an age-friendly interface, aggregating patient responses, voice biomarkers, and remote patient monitoring data to enable preventative and proactive healthcare.[4]

The platform is designed to reduce the administrative burden on clinical staff, particularly nurse practitioners who may spend significant time preparing outreach lists, navigating between electronic health record systems, and gathering chart data before they can begin closing care gaps. Kaigo Health's tools automate tasks such as ADT (admission, discharge, and transfer) follow-ups, prior authorizations, care gap identification and closure, claims auditing, and the capture of HCCs and CPT codes. The company also aims to improve quality metrics including CAHPS and HCAHPS scores.[5]

Kaigo Health is based in the San Francisco Bay Area and operates in the health technology, digital health, and healthcare IT sectors. The company was among eight startups founded or co-founded by University of Illinois Urbana-Champaign students and alumni in Y Combinator's Fall 2025 cohort, a group highlighted for bringing AI innovation to industries including healthcare.[6]
